Medicaid Citizenship and Immigration Eligibility Changes Beginning October 1, 2026 

Federal Medicaid rules related to citizenship and immigration eligibility are changing beginning October 1, 2026. Some Medicaid recipients may receive a letter, renewal form, or request for information from the Alaska Division of Public Assistance (DPA). 

Receiving a notice does not automatically mean your Medicaid coverage is ending. 

Frequently Asked Questions

  • The change may affect some people who qualify for Medicaid based on certain citizenship or immigration categories, including some refugees, asylees, parolees, trafficking victims, and other non-citizens. 

    Some lawful permanent residents (green-card holders), Cuban/Haitian entrants, and COFA migrants may continue to qualify if they meet other Medicaid requirements.

    • Read the entire notice and check any deadlines. 

    • Respond by the deadline if DPA asks you to renew your coverage or provide additional information.

Medicaid Changes Beginning in 2027 

Some Medicaid rules are changing in 2027, including how often eligibility is reviewed for many adults in the Medicaid expansion group and new work and community engagement requirements for some expansion adults.

If you received an advance notice about these changes, it does not mean you are losing Medicaid. Your coverage has not changed because you received the notice, and you do not need to respond to the advance notice itself.

  • There are two separate changes planned for adults in the Medicaid expansion group:

    More frequent eligibility reviews: Many adults ages 19–64 in the Medicaid expansion group will have their Medicaid eligibility reviewed every 6 months instead of every 12 months.

    Work and community engagement requirements: New requirements may apply to some adults ages 19–64 in the Medicaid expansion group.

  • No. A 6-month review means the State will check your Medicaid eligibility more often. It does not mean your coverage automatically ends every 6 months.

    Alaska Native and American Indian individuals and people eligible for Medicaid based on circumstances such as age, disability, or pregnancy will continue on a 12-month review cycle.

  • No. The requirements apply only to some adults ages 19–64 in the Medicaid expansion group.

    People enrolled in other Medicaid eligibility groups are not subject to these requirements.

    The requirements are currently scheduled to begin January 1, 2027, but Alaska has requested additional time to implement them.

  • You may not have to meet the requirements depending on your circumstances. Exclusions may apply to:

    • Alaska Native and American Indian individuals;

    • pregnant or postpartum individuals;

    • certain parents and caregivers;

    • people who are medically frail or have qualifying serious medical needs;

    • people participating in qualifying drug or alcohol treatment;

    • certain people already meeting other public-benefit work requirements;

    • certain veterans with a total disability;

    • certain former foster care individuals under age 26;

    • and certain incarcerated individuals.

    Other exceptions and temporary hardships may also apply, including circumstances involving Medicare enrollment during the review period.

  • If you received the advance notice:

    • You do not need to respond to the advance notice itself.

    • Keep your contact information current with the Division of Public Assistance.

    • Watch for future Medicaid notices.

    • If you receive a renewal or a letter asking for information, respond by the deadline shown.

Marketplace & HealthCare.Gov Q&A Sessions

Need help with your health insurance Marketplace application or have questions about HealthCare.gov? ANHC is offering free, weekly question-and-answer sessions during open enrollment. Stop by to get help with specific questions about applying for or enrolling in health insurance.

We can help answer questions about:

  • Marketplace health insurance coverage

  • Accessing your HealthCare.gov account or resetting your password

  • Documents needed to apply

  • Eligibility for coverage

  • Completing your application or enrolling in a plan

These sessions are best for people who have specific questions and do not need a full, one-on-one appointment.

No appointment needed. Stop by on Fridays from 3:00–4:30 PM. We’re located at 4951 Business Park Blvd, Anchorage (3rd floor).

Medicare Assistance

Have questions about Medicare or need help understanding your coverage?

ANHC can help you understand your Medicare benefits and find out if you qualify for programs that may help lower your healthcare costs, including:

  • Medicare Savings Program (MSP): Helps eligible individuals pay certain Medicare costs.

  • Extra Help: Helps eligible individuals pay for Medicare prescription drug coverage.

  • Medicaid for Seniors: Provides additional healthcare coverage for eligible older adults.

Please note: ANHC can answer questions and provide guidance, but we cannot enroll you in Medicare Parts A, B, C, or D or help you enroll in a Medicare Part D prescription drug plan.
