Remember that your Sliding Fee Scale Discount applies to all services at ANHC, including dental care and prescription medications from our pharmacy.

How to Become a Patient at ANHC

1. Complete a New Patient packet

Online Registration Option

Call or send TEXT ME to 907-743-7200 to register as a new patient and schedule your first appointment.

Within 1-2 business days, you will receive a text with links to complete the following forms:

  • New Patient Packet

  • Medical Health History Form

  • Sliding Fee Scale Discount Application

Need a printed packet mailed to you? Call 907-743-7200 to request one. Once completed, you can return the packet in person or by mail to 4951 Business Park Blvd., Anchorage, AK 99503.

Within 1-2 business days, you will receive a call to schedule your first appointment.

Important Note

If you have not scheduled your first appointment or have not been seen by an ANHC provider in the last 36 months, you are considered a new patient.

2. Apply For the Sliding Fee Discount Program

Our Sliding Fee Scale Discount Program helps reduce the cost of health care for eligible patients. All patients are encouraged to apply, even if you have insurance.

Before your first visit, please review our patient policies and important notices.

4. Attend your first appointment

Please bring these items with you to your first appointment:

  • ​Insurance cards

  • Current address and phone number

  • Current medications

  • Medical records

  • Paperwork for your provider (disability, CDL, etc.)

  • Proof of income (if applying for the Sliding Fee Scale Discount Program)

  • Questions you have

  • Payment for your care

Your first appointment may last up to 45 minutes. During this visit, you and your provider will discuss your immediate concerns, your health history, and your health challenges and goals.

  • Sliding Fee Discount Program: Be sure to apply for the Sliding Fee Scale Discount Program if you haven’t already.

  • Health Insurance Enrollment Assistance: Our Outreach, Enrollment, & Eligibility team can help you sign up for health insurance, including Medicare and Medicaid.

  • Prices of Common Services: Find a list of prices of common services and request an estimate for medical and dental care.

  • Payment Information: We accept all forms of insurance, including Medicaid and Medicare. We also accept cash, check, and major credit and debit cards. If you are paying by credit or debit card, you may pay online. Learn more about our billing practices.
