New patients must complete a New Patient Packet before scheduling their first appointment. Step by step instructions are below.

If you have not been seen by a provider at ANHC in the last 36 months, you are considered a new patient.

1. Complete the New Patient Packet.

2. Submit your New Patient Packet.

3. Schedule an appointment.
After we receive your New Patient Packet, we will contact you within one business day to schedule your first appointment. If you haven't heard from us within that time, please give us a call.

4. Learn about Luma Health.
Luma Health is a way to communicate with your ANHC care team. Use text messaging and a secure patient portal to make appointments, check in when you arrive, complete paperwork, receive lab results, request prescription refills, and more. Learn more about Luma.

5. Attend your first appointment.
Bring these items with you:

Payment for your visit

Your current address and phone number​

Proof of income if applying for the Sliding Fee Discount

Previous medical records and immunization records

Any additional paperwork or forms for your provider (disability, CDL, etc.)

Your first appointment may last up to 45 minutes.
​
You and your provider will address your immediate health concern, discuss your health history, and talk about your overall health.

5. Billing
ANHC accepts all insurance plans, including private health insurance, Medicaid, Denali KidCare, Medicare, and TriCare. We also offer a sliding fee discount program to eligible patients.
