New patients must complete a New Patient Packet before scheduling their first appointment. Step by step instructions are below.

If you have not been seen by a provider at ANHC in the last 36 months, you are considered a new patient.

1. Complete the New Patient Packet.

3. We'll contact you within 1 business day to schedule an appointment.

After we receive your paperwork, we will send you a text or give you a call within 1 business day to schedule an appointment. If you haven't heard from us within that time, please give us a call.

4. Learn about Luma Health.

Luma Health is a way to communicate with your ANHC care team. Simply send a text to make appointments, check in when you arrive, complete forms, receive lab results, request prescription refills, and more. Learn more about Luma.

5. Attend your first appointment.

Bring these items with you:

  • ​Insurance cards and medical coupons (if any)

  • Current medications

  • Payment for your visit

  • Your current address and phone number​

  • Proof of income if applying for the Sliding Fee Discount Program. (If you applied online, you already provided this information.)

  • Previous medical records and immunization records

  • Any additional paperwork or forms for your provider (disability, CDL, etc.)

Your first appointment may last up to 45 minutes.

You and your provider will address your immediate health concern, discuss your health history, and talk about your overall health.

5. Billing

In addition to our Sliding Fee Discount Program, ANHC accepts all insurance plans, including private health insurance, Medicaid, Denali KidCare, Medicare, and TriCare. We can help you sign up for health insurance and public assistance programs.
