If you have not been seen by a provider at ANHC in the last 36 months, you are considered a new patient.
To become an ANHC patient:
1. Complete the New Patient Packet.

2. Submit your New Patient Packet.

3. After we receive your application, you should be contacted within one business day to schedule your first appointment. If you haven't heard from us within that time, please give us a call:
For medical appointments, call (907) 743-7201.

4. Bring these items with you to your first appointment:
Insurance cards and medical coupons (if any)
Payment for your visit
Your current address and phone number
Proof of income if applying for the Sliding Fee Discount
Previous medical records and immunization records
Any additional paperwork or forms for your provider (disability, CDL, etc.)
Your first appointment may last up to 45 minutes.
You and your provider will address your immediate health concern, discuss your health history, and talk about your overall health.
ANHC accepts all insurance plans, including private health insurance, Medicaid, Denali KidCare, Medicare, and TriCare. We also offer a sliding fee discount program for eligible patients.
