Care for All

Complete a New Patient Packet and Sliding Fee Scale Discount Application, and you’re on your way to comprehensive primary care for the whole family.

Remember that your Sliding Fee Scale Discount applies to all services at ANHC, including dental care and prescription medications from our pharmacy.

1. Complete a New Patient packet. 

If you completed a paper form, return the completed copy in person or by mail. Our physical/mailing address is 4951 Business Park Blvd., Anchorage, AK 99503.

We’re also happy to mail you a New Patient Packet. To request a packet, call 907-743-7200 or send an email to scheduling@anhc.org.

The New Patient Packet includes the following forms:

  • New Patient Registration

  • Consent for Treatment and Billing Practices

  • PHI Disclosure Agreement

Note: If you have not been seen by an ANHC provider in the past 36 months, you are considered a new patient.

 

2. Complete a Sliding Fee scale Discount Application.

Our Sliding Fee Scale Discount Program reduces the cost of health care for eligible patients. All patients are encouraged to apply for the Sliding Fee Discount Program, even if you have insurance.

3. read these important documents.

4. schedule your first appointment.

After we receive your New Patient Packet, we’ll send you a text or give you a call within one business day to schedule an appointment. If you haven't heard from us within that time, please give us a call.

5. Attend your first appointment.

Please bring these items with you to your first appointment:

  • ​Insurance cards

  • Current address and phone number

  • Current medications

  • Medical records

  • Paperwork for your provider (disability, CDL, etc.)

  • Proof of income if applying for the Sliding Fee Scale Discount Program. (If you applied for the Sliding Fee Discount Program online, you already provided this information.)

  • Questions you have

  • Payment for your care

Your first appointment may last up to 45 minutes.

You and your provider will discuss your immediate concern, your health history, and your health challenges and goals.

Sliding Fee Discount Program: Be sure to apply for the Sliding Fee Scale Discount Program if you haven’t already.

Health Insurance Enrollment Assistance: Our Outreach team can help you sign up for health insurance, including Medicare and Medicaid.

Prices of Common Services: Find a list of prices of common services and request an estimate for medical and dental care.

Payment Information: We accept all forms of insurance, including Medicaid and Medicare. We also accept cash, check, and major credit and debit cards. If you are paying by credit or debit card, you may pay online. Learn more about our billing practices.
