If you have not been seen by a provider at ANHC in the last 36 months, you are considered a new patient.

To become an ANHC patient:

Call 743-7200 if you would like the packet mailed to you.

Then submit your New Patient Packet.

Next, call us and make an appointment.

For medical appointments, please call (907) 743-7201.

For dental appointments, please call (907) 743-7202.

When you arrive for your first appointment, please bring these items with you:

  • ​Insurance cards and medical coupons (if any)

  • Current medications

  • Payment for your visit

  • Your current address and phone number​

  • Proof of income if applying for the Sliding Fee Discount

  • Previous medical records and immunization records

  • Any additional paperwork or forms for your provider (disability, CDL, etc.)

Your first appointment may last up to 45 minutes.

You and your provider will address your immediate health concern, discuss your health history, and talk about your overall health.

ANHC accepts all insurance plans, including private health insurance, Medicaid, Denali KidCare, Medicare, and TriCare. We also offer a sliding fee discount program for eligible patients.
