ANHC provides health care, education, and support for those who are at risk for diabetes or who are currently living with diabetes. We’ll make sure you have the information and tools you need to manage your condition and stay well.

About Diabetes

Diabetes is a condition that happens when your body can’t process sugar normally. Too much sugar stays in your bloodstream, and this extra sugar can cause serious health problems, like heart disease, vision loss, and kidney disease.

To prevent and manage diabetes:

  • Eat healthy food.

  • Stay active.

  • Stay informed.

  • Watch your weight.

  • Take medicine if your provider prescribes it.

  • Ask your provider about Omada.

If you have prediabetes, enroll in ANHC’s Diabetes Prevention Program.

Could you have prediabetes? Take the Prediabetes Risk test. (Available in Spanish.)

 
 

Diabetes Prevention Program

ANHC’s Diabetes Prevention Program (DPP) class is for individuals who have prediabetes. If you have prediabetes, your blood sugar levels are high but not high enough to be diabetes.

(Take the Prediabetes Risk Test in English and Spanish.)

ANHC’s next DPP class series:

  • Will have English and Spanish class options.

  • Is free for all participants.
